چکیده

Right atrial scars participating in macro reentrant tachycardia circuits are generally seen in patients following cardiac surgery for congenital heart disease. Right and left atrial scars without any prior cardiac surgery have been reported but are rare [1,2]. A 65 year old farmer had recurrent drug refractory atrial tachycardia for which he underwent an EP study with three dimensional mapping and radiofrequency ablation. He was not a diabetic or hypertensive, had a structurally normal heart with preserved biventricular function, and normal coronaries. He had not undergone any cardiac surgery prior. Three dimensional mapping was done using CARTO 3 (Biosence, Webster). Sinus rhythm mapping of the right atrium (RA) showed a large atrial scar, defined as electrically silent areas with no recordable activity or bipolar voltage amplitude < 0.25 V. The scar was distributed over posterior, anterior and anterolateral areas near the tricuspid annulus (TA). Mapping during tachycardia showed macro reentrant tachycardia (cycle length =370 msec) propagating around RA scar. Figure 1 shows the sinus rhythm bipolar voltage map of the right atrium (RA) shows the large extent of the RA scar. Figure 2 shows the late split atrial potentials mapped at the scar borders. Figure 3 shows activation map of atrial tachycardia, demonstrating "Early meets late" at the upper scar border.

Recurrence of atrial fibrillation correlates with the extent of post-procedural late gadolinium enhancement: a pilot study.

OBJECTIVES We sought to evaluate radiofrequency (RF) ablation lesions in atrial fibrillation (AF) patients using cardiac magnetic resonance (CMR), and to correlate the ablation patterns with treatment success. BACKGROUND RF ablation procedures for treatment of AF result in localized scar that is detected by late gadolinium enhancement (LGE) CMR. We hypothesized that the extent of scar in the ...

Combining acquisition and image processing methods to improve evaluation of arial wall scar patterns after pulmonary vein isolation

Background Pulmonary vein isolation (PVI) is a therapy to treat patients with atrial fibrillation (AF) but suffers a >30% recurrence rate. Studies have shown that gaps in the scar pattern encircling the pulmonary veins are linked to recurrence. Co-registering angiography images of the atrial blood pool to late gadolinium enhanced (LGE) images of the atrial wall to determine scar pattern suffer ...

Right atrial tachycardias related to regions of low-voltage myocardium in patients without prior cardiac surgery: catheter ablation and follow-up results.

AIMS There are only few descriptions of patients without prior cardiac surgery in whom a large low-voltage zone (LVZ) or scar is the electrophysiological substrate for various atrial tachycardias. We describe the electrophysiological and electroanatomic characteristics of unusual macroreentrant atrial tachycardias (MRATs) in seven patients with spontaneous right atrial (RA) scarring and present...

Simplified progressive approach for the ablation of scar related atrial macroreentrant tachycardias.

INTRODUCTION Radiofrequency ablation of scar related right atrial flutter is challenging. Long procedures, prolonged fluoroscopic times and high percentages of recurrences are of concern. We present a simple and progressive approach based on a single electroanatomic map of the right atrium. METHODS Twenty-two consecutive patients with atrial flutter and history of cardiac surgery were include...

Idiopathic dilatation of the right atrium. A case report.

Idiopathic dilatation of the right atrium in a young woman is reported. Thoracotomy was performed for suspected pericardial cyst. Intraoperative findings suggested idiopathic right atrial dilatation, which was histologically confirmed. Two years after partial resection of the dilated right atrial wall the patient is doing well. Management of asymptomatic idiopathic dilatation of the right atriu...
